Reputation is just a fun addition to the OH storyline. Every character gains a reputation of one sort or another as his career progresses, expressed as Reputation Points. While a character might try to take advantage of his reputation from time to time, usually the character's reputation precedes him - whether he wants it to or not.
Higher reputations can sometimes give you boosts on adventures, as NPC's will be more likely to help you and your group if they recognize the character and they have a positive reputation. A high reputation bonus makes it difficult for a character to mask his identity though, which can be a problem if he's trying not to be noticed.
DM's may give reputation points as awards along with XP, GP and loot at an adventure's conclusion. This would represent how much more famous (or infamous) their recent actions have made them. Points can go up by .5 or 1 point at a time - very rarely 1.5 for high level quests.
You are allowed to customize your character's reputation by choosing what bonuses you gain for your reputation. A hero might choose Diplomacy, and Gather Information for bonuses, and not be quite as good at being Intimidating. This would be considered "Fame". Someone more chaotic or even evil aligned might decide that being infamous and scaring people into giving them what they want works better, and will take the boost in Intimidate and Bluff, which would be considered "Infamy". You can choose as you wish.
Nom de plumes and Secret Identities: If you successfully pass a Disguise check, then anything you do while under that secret identity or using a fake name does not effect your reputation. Some may even choose to keep up with the fame or infamy of their secret identity.
Bonuses and Penalties: If you do not wish to take the penalties, then just don't take the bonuses. You can skip them if you like.
